whats does a social prescriber do?

hi! i went to mt GP about anxiety (mainly social anxiety) symptoms (unsure if its POTS or just anxiety) and i got referred to a social prescriber, but what do they actually do? is it worth contacting them? ive never even heard of it until now, thank you!!
