What would be causing a transformation mod to only have 1 of the forms instead of all 3 that are in the mod?

I've downloaded a mod that is supposed to have 3 forms but when I use the skill it only has one which is the last one. The only file that comes in the mod is the xv2 file that was installed with the installer. I'm not sure what else could cause the issue. Anyone that is familiar with adding transformation mods to PC would be greatly appreciated
