I need a letter from a medical professional stating that my change of gender is likely to be permanent...

I live abroad, so I don't have a GP. My doctor won't touch this, I don't have access to a psychiatrist or anything via the health service here. But I can go private. Does anyone have any advice? I can't travel (passport is with passport people...) but can do video calls etc. Thanks in advance!
